Accessed by referral from Child & Youth Mental Health (MCFD) whereby outreach counsellors assist and support children, youth and families with ongoing emotional and/or behavioral issues through outreach and counselling activities that promote healthy functioning within the home and community environment.

Transitions is designed to help youth and their families with problematic drug and alcohol use achieve their goal of abstinence or reduce harmful consequences of substance misuse.
