Home Health Monitoring for Heart Failure

Home Health Monitoring is a tool to help you monitor your heart failure in your home. After an education session with a nurse, you will be provided with an easy-to-use tablet, weigh scale, blood pressure machine and a oxygen measuring tool which are installed in your home by a technician.

You will use the equipment each day for about 3 months to increase awareness of your disease and when to take action to prevent  hospital admissions. The information, including blood pressure and weight, is sent  to the nurse, who monitors the information and  changes in your health status. The nurse will work with you over the phone, helping you understand changes in your condition.  The entire process takes only a few minutes a day. It is secure and all of your information is confidential.  The information is also shared with your family physician.
