Self-Management Health Coach program
Self-Management Health Coach Program provides individualised telephone support for people living with chronic diseases such as diabetes, chronic pain, cancer, heart, kidney and lung diseases, arthritis, lupus, Parkinson, MS, etc. Volunteer health coaches provide weekly 30 minutes telephone coaching for 3 – 6 months for individuals wanting to improve their confidence and motivation to become healthier and to better manage the daily challenges of living with chronic conditions. Individuals are paired with coaches based on gender and possibly shared chronic conditions. more info

Training and support for adults with developmental disabilities who live on their own. Teaches skills required to live independently.more info

Provides counselling and support to alleviate loneliness and isolation of seniors who are age 55+. The service is offered in the home, the office and community. Seniors are encouraged to enjoy healthy ageing through physical, emotional, and spiritual well-being.more info
